During flight, at 1915hrs, the starboard motor coughed four times, then picked up until 1930hrs when it started to run roughly, then at 1936hrs started backfiring at an altitude of 8000 feet at location 06º 44' South/162º 09' East, some 120 nautical miles from Kiriwina. A/c now on port engine with bombs being already jettisoned, with the starboard engine having seized at 1948hrs and airscrew feathered, the a/c diverted to Kiriwina. Some 180 gallons of fuel in the outer fuel tanks jettisoned. The a/c was still losing height and so at 1955hrs the crew were ordered to jettison approximately 1000lb of ancillary equipment to lighten the load which arrested the height loss. The a/c landed without any further issues on the Sia Aerodrome at Kiriwina without any further damage. The navigation aids were rendered unserviceable and on return to base, the a/c ran short of fuel before locating base an d was ditched in Collingwood Bay at 2430hrs. They were rescued by a native dugout canoe which towed them back to the shore. The following morning, they were met by a large outrigger canoe that had set out from Tufi, a small remote ANGAU base. After operating the fixed fire extinguishers in flight, one landing the nacelle again caught fire until the Duty Fire tender arrived and extinguished the fire. The Duty Pilot had already left his post to check on another landed aircraft's number. Pilot of A9-336 immediately taxied to the edge of the runway and stopped, still facing the same direction. He had seen that the Beaufort had moved off the strip and stopped to make room. The Vengeances (A27-277, A27-85 and A27-274) then made their approach in intervals of 300 yards, with the first Vengeance landing short and to the left-hand side of the strip and passed the Beaufort at taxy speed with plenty of clearance. The second Vengeance(A27-85) landed further up the runway but appeared to veer to A9-336 and eventually struck it with its starboard wing at between 20-30 mph damaging the center section of A9-336. The third Vengeance landed on the other side of the strip and was clear. A half hour into the exercise, the pilots of these two trailing a/c noticed the starboard elevator trim tab on A9-346 had become free and was flapping. The pilot of A9-346 radioed that was causing a violent vibration to the entire elevator surface. He advised that he would proceed to an Allied Naval Vessel (USS Chanticleer) approximately 5 miles in distance. When the a/c got within 1 mile of this vessel and at a height of 1000 feet, the a/c commenced diving (The pilot of A9-343 noticed that the elevator trim tab had jammed into nose down position and as such the a/c continued to dive at an increase angle until impacting into the sea at a final angle of 70º, some 18 miles northwest of Rottnest Island. Crew: Pilot: Wg Cdr Charles Cuthertson Learmouth DFC and Bar Serv#385, Nav(B) F/O Gordon Gwynne Moore Serv#401523, WAGs P/O Frederick Erick Chidlow Serv#406689 and P/O Douglas Raymond Cullen Serv#406095 were posted missing believed killed. The USS Chanticleer (Submarine Rescue Ship assigned in support of USN Submarines based at Fremantle. Served in US Navy 1942 to 1973) anchored over the site, along with a US Navy Submarine and had sent down a diver who at 132 feet of depth located the wreckage which was in two parts. Only one crew member was sighted, but he was not retrieved. The following day when the airframe was being salvaged, no bodies remained on site.
